The analysis of individual-level seroprevalence's risk factors employed a logistic regression model. The mortality experience on the kidney transplantation waiting list varies across racial and ethnic groups in the United States, but the reasons behind this remain unclear. This study aimed to determine whether disparities exist in the predicted outcomes for kidney transplant (KT) candidates on the waiting list, considering their race and ethnicity, in the contemporary US setting.
We contrasted in-hospital mortality or primary nonfunction (PNF) rates between waiting-list and early posttransplant periods for adult (18 years of age) white, black, Hispanic, and Asian kidney transplant (KT) candidates in the United States from July 1, 2004, through March 31, 2020.
Among the 516,451 participants, 456%, 298%, 175%, and 71% represented the white, black, Hispanic, and Asian demographics, respectively. A 3-year waiting list, encompassing patients removed for worsening conditions, exhibited substantial racial disparities in mortality, with rates of 232%, 166%, 162%, and 138% among white, black, Hispanic, and Asian patients, respectively. Among transplant recipients, the proportion of in-hospital deaths (PNF) attributed to kidney transplants (KT) was 33% for black patients, 25% for white patients, 24% for Hispanic patients, and 22% for Asian patients. Among transplant candidates, white individuals showed the highest risk of death during the waiting period or while becoming too ill for a transplant. Conversely, black (adjusted hazard ratio, [95% confidence interval], 0.67 [0.66-0.68]), Hispanic (0.59 [0.58-0.60]), and Asian (0.54 [0.52-0.55]) candidates demonstrated a lower mortality risk. Compared to white patients, Black kidney transplant (KT) recipients displayed a markedly increased risk (odds ratio, [95% CI] 129 [121-138]) of death or post-operative complications before discharge. Controlling for confounding variables, Black recipients (099 [092-107]) exhibited a comparable, elevated risk of post-transplant in-hospital mortality, or PNF, similar to white recipients and distinct from Hispanic and Asian recipients.
